In the fourth week of pregnancy, the embryo implantation in the uterus would already occur in most women. This may be accompanied by implantation spotting, which is a typical symptom of the fourth week of pregnancy. Since the egg has implanted itself in the soft, expanding mucous membrane, the embryo can more easily take up the nutrients it now needs for dynamic growth. It also begins to produce an important hormone necessary for the normal development of early pregnancy – human chorionic gonadotropin, or beta-HCG. At the same time, the production of another hormone – progesterone responsible for maintaining pregnancy – also increases. The higher levels of these hormones may lead to the first subtle signs of pregnancy before missed period, which include:

- breast tenderness and soreness,

## When to see your doctor

For the above reasons, one could conclude that a late period does not mean anything bad. In fact, in some cases it is perfectly normal. However, it is worthwhile in all cases to seek medical advice. Particularly if your period delay is repeated every month or if you have not had your period for two months. Rarely it can be a symptom of a serious illness, such as cancer or diseases of the reproductive system. Also, don't delay seeing your doctor if you have other complaints: severe pain, excessive hair loss, vaginal spotting, acne (which you cannot control), being overweight or underweight.

When you do get your first period postpartum, you may experience cramping as your uterus shrinks back to a smaller size and its lining has more cells to shed. This should decrease each month as your body recuperates.

Expect to see blood and vaginal discharge in the weeks after giving birth as your body releases fluids it no longer needs to protect your baby. They will be red at first, but they should lighten over time. This is called the ‘lochia’ and reflects the final shedding of the placenta and any pregnancy tissue within the uterus which has not been shed at birth – this is normal and nothing to be worried about. If you see blood six weeks or more after birth, it is likely to be your first period postpartum.

Your first period may or may not be different from what you experienced before pregnancy. It can be irregular and it can last anywhere from a few days to a full week. However, it is worth reporting this fact to your doctor. Frequent spotting during lactation or between menstrual bleeds also requires medical consultation.

While white-coloured discharge is totally normal, texture-wise it should be the same all the way through, without lumps – whether it is sticky, gooey, thin, elastic or thick. This alternative version with clumps in it is not so great, and is very rarely going to be a positive discovery.
